BB PLAYERS REGISTRATION FORM

All performers at Broadway Bound Players must have a completed REGISTRATION FORM. This REGISTRATION process helps us to keep up-to-date records and contact information, as well as CONSENT and RELEASE of LIABILITY. If you have any questions, please send an email susan.bbplayers@mac.com.

*MEDICAL AUTHORIZATION: I/We the parent(s) of the above named minor, know that I may not be available to authorize the medical care of said minor child and appoint Broadway Bound Players to act in my absence to give such authorization. Broadway Bound Players staff and faculty have my consent to authorize emergency medical care.

*CONSENT AND RELEASE FROM LIABILITY: I/We the parent(s) of the above named minor performer, for and in consideration of such Performer's participation in the instructional and recreational programs of BROADWAY BOUND Players (BBP), do herewith and hereby agree to indemnify and hold harmless the said BBP, it's Board of Directors, Creative staff and volunteers, from any and all liability, loss or damage, including reasonable attorney's judgements against the said BBP, it's Board of Directors, Creative Team, Staff and Volunteers, from whatsoever extent of nature, including without limitation, an injury, illness or accident, to such Performer arising from such Performer's participation in any program or course of instruction of the said BROADWAY BOUND Players.

*FEES AND FURTHER RESPONSIBILITIES: I/We understand that any fees paid to BROADWAY BOUND Players are non-refundable. I/We also understand that further fees may be collected for items such as costumes, cast photos, program ads and tributes, flowers and other items. Broadway Bound Players may also ask each family to volunteer and/or donate snacks for some, but not all, or our productions as needed.

*PUBLICITY PHOTO RELEASE: BROADWAY BOUND Players has my permission to use photograph or my child's photo graph in media to support the informational and promotional purposes of BROADWAY BOUND Players.

*AUDITIONS: I/We understand that some programs of BROADWAY BOUND Players are available for participation based on the results of an AUDITION process. These programs are designed to create high levels of performance and are something to aspire to. By checking this box, I understand that my child may or may not be included in part of the audition only productions, and I understand that the results of the audition selections are final.
